Application Instructions

1. Complete this two-page application form, including the required information on the two recommendation forms.

2. Write an original essay of about 500 words on the following:
As a young woman, identify an issue about which you feel strongly.
Explain your choice and discuss the opportunities that you have had to address this issue or learn more about it.

3. Be sure your name is on all pages of your application.

4. Attach two recommendation forms and letters, one from a school representative, the other from someone other than a
family member. Ask each person to complete the form and write a letter of recommendation. The letter should explain
fully why you should be selected for this leadership program. We ask that you request your letters early so as not to
delay the submission of your application. The recommendation forms and letters should be returned to you in a sealed
envelope or mailed directly to the Eleanor Roosevelt Center at Val-Kill.

 

5. Mail your complete application, including the two sealed recommendation forms and letters, to:

Incomplete applications will not be reviewed.

Deadline for receipt of applications is March 1st, 2004.
Participants will be notified by April 14, 2004.

PLEASE NOTE:

Selected girls pay a $500 registration fee and are responsible for their transportation to and from the GLW Workshop. Financial assistance is available upon request.
